- Download and fill out the following forms (please note you will use the rate quotation form for 100+ group if you have more than 100 employees):

- Group Appraisal Form [File: 65 KB]

- Small Business Fax Cover Page [File: 54KB]
- 10-99 Rate Quotation Checklist
- Fax the four forms listed above to the Blue Cross and Blue Shield of Oklahoma Underwriting Department at (918) 551-3735.
- You will be contacted when your proposal is complete, usually within 5-7 working days.
