BlueLincs HMO has introduced innovations to help keep premiums affordable for both employers and health plan members — BlueLincs' three lowest-priced health plans now have options that include annual deductibles. 

BlueLincs Plan H, Value Option and Special Option now are available to employer groups with choices of $200, $300, $500 and $1,000 annual deductibles.

The lowest premiums of any BlueLincs HMO plan.
Primary care physician office visits fully covered, with just a $20 office visit copayment.
Specialist office visits fully covered, with just a $30 office visit copayment.
No deductible for hospitalization, doctor's office visits, emergency care or any other covered health care service. A $300 deductible applies only to prescription drug benefits.
30 percent copayments for most other health care services.
The annual maximum out-of-pocket amount a member pays under BlueLincs Special Option is $3,000.

Deductible plans now available for BlueLincs Value Option — save even more on premiums! Plans with deductibles of $200, $300, $500 and $1,000 are available.

Other features for all BlueLincs Value Option plans include:

Preventive care fully covered, with just a $20 office visit copayment.
No deductible for hospitalization, doctor's office visits, emergency care or any other covered health care service. A $300 deductible applies only to prescription drug benefits.
20 percent copayments for most other health care services.
The annual maximum out-of-pocket amount a member pays under BlueLincs Value Option is $2,000.
