Open enrollment for Blue Pathway^SM child-only policy runs through July 31, 2012

Blue Pathway — our health insurance plans for children who do not have other major medical coverage available to them for purchase.

• Plan for applicants ages 1 through 18 years old¹
• Coverage for emergency care, office visits, hospital stays and prescription drugs
• Preventive care services² covered at 100%

Blue Pathway provides guaranteed acceptance for applicants who are least one year old and under 19 years of age.

¹ Children eligible for these plans will be approved for coverage, regardless of health status. Applicants must still meet any non-health related eligibility criteria and must not have other major medical coverage available to them for purchase.

² Benefits covered as defined by national guidelines. Must use in-network providers to receive maximum coverage.
