The ultimate in choice, Standard Option allows you to receive services from Preferred and Non-preferred providers.

With a network of more than 400,000 doctors, dentists, hospitals and pharmacies worldwide, we make it easy for you to select one of our Preferred providers. But if your current doctor is not one of our Preferred providers, no problem. Standard Option gives you the freedom to use any doctor, or physician assistant. Most care is covered at 90 percent PPA (preferred provider allowance) and you pay only $15 for office visits. You'll also receive 75 percent prescription drug coverage at Preferred retail pharmacies. The affinity drug program will provide you discounts on some prescription drugs without additional premiums. And if you have your prescriptions filled by mail order, you pay only a $10 copay for generics and a $35 copay for name brands.

Add Federal DentalBlue Standard for extra care.

Federal employees selecting Standard Option can subscribe to Federal DentalBlue Standard. Federal DentalBlue Standard is a supplemental dental plan offered by Blue Cross and Blue Shield plans across the country. Thousands of Oklahoma's Federal Employee Program members have already chosen Federal DentalBlue Standard.

A smaller benefit applies to out-of-network services, so to receive your maximum benefits, always use preferred provider networks.

This cost-effective dental plan offers federal employees 100 percent coverage for:

Basic Option

Basic Option gives you the freedom to see any doctor in our Preferred network nationwide with no referral, no claim forms to file and no deductible to pay. Beginning in 2007, you can enjoy no premium increase from 2006, and many other benefits Basic Option has to offer:

• $20 for office visits
• 100% coverage for many preventive services, including pediatric care
• $10 copayment for generic prescription drugs at Preferred retail pharmacies
• Affinity drug program — discounts on some prescription drugs at no additional premium
• $20 chiropractic visits (up to 20 spinal manipulations a year)

Federal employees selecting Basic Option can subscribe to Federal DentalBlue Basic. Federal DentalBlue Basic is a supplemental dental plan that enhances the dental benefits you already get under Basic Option, and includes paying for your $20 copay. You may enroll in DentalBlue anytime, not just during open season.

A smaller benefit applies to out-of-network services, so to receive your maximum benefits, always use preferred provider networks.

This cost-effective dental plan offers federal employees:

80 percent on basic restorative procedures

• Fillings
• Extractions
• Pin retention

50 percent on major restorative procedures

• Root canals
• Crowns
• Bridges

Many other dental services also are covered.

Blue Vision
Basic Option and Standard Option members can also choose additional coverage on vision products and services at substantial savings from Blue Vision network providers:

• Routine eye exams
• Frames
• Lenses
• Conventional contact lenses
• Laser vision corrrection

Members have access to more than 25,000 providers nationwide, including independent optometrists, ophthamologists, and many of the nation's premier retail centers.

Discount vision program
Both Basic Option and Standard Option members can enjoy the Federal Employee Program discount vision program.
