  •  Prescription Drug Formulary1 - Effective April 1, 2008 - A multi-tier formulary is available for members with a prescription drug card that includes copayments. The generic tier will cost the member the least out-of-pocket expenses. If a prescription drug is not listed on the formulary, it is considered to be on the brand name tier.  [File: 165 KB]

  • Maintenance Drug List - Some medications are designated as maintenance prescription drugs and may have quantity limits. Refer to your benefit plan for detailed information on coverage of maintenance prescription drugs.  [File: 50 KB]
  • Prescription Drugs with Quantity Limits - Prescription drug medications with monthly quantity limits. To verify prescription drug benefits, contact the Customer Service number on your member ID card.  [File: 112 KB]
  •  Drug Card Prescription Claim Form - Submit prescription medication claims for drug card reimbursement.

Financial assistance available for prescription drugs

HelpingPatients.org, is a Web site maintained by the Pharmaceutical Research and Manufacturers of America (PhRMA) and 48 of its member companies. HelpingPatients.org is designed to help health care consumers find patient assistance programs for which they may qualify.

Patient assistance programs can help patients receive financial assistance in acquiring their needed medicines. HelpingPatients.org's online service is free and confidential.
