Blue Access for Employers

2016 Summary of Benefits & Coverage and Plan Brochure

To help you make an informed decision about your health care coverage, a Summary of Benefits & Coverage (SBC) document is available for each of our plans. The SBC describes key features such as:

  • Covered benefits
  • Cost-sharing provisions
  • Coverage limitations
  • Exceptions

We offer four categories, or levels, of plans: bronze, silver, gold and platinum. Each level offers the same set of essential health benefits, quality and amount of care. The differences are how much your premium costs each month, what portion of the bill you pay for things like hospital visits or prescription medications, and how much your total out-of-pocket costs are.

Starting in 2016, pediatric dental coverage is included in all small group qualified health plans as an essential health benefit embedded into the medical plan. View 2016 Embedded Pediatric Dental Benefit for Medical Plans.

Information in these SBCs represents an overview of coverage. It is not a complete list of what is covered or excluded. Information is subject to change. The full terms of coverage are located in the insurance policy.
