Blue Cross and Blue Shield of Oklahoma offers numerous Health Management programs to address your medical and behavioral health needs.

What Is the Case Management Program?
The Case Management program provides you with your own health advisor who will help you with questions about your doctor's treatment plan and send you tips about managing your health along the way. The program aims to help:
- Keep you healthy
- Help manage your emerging health risks
- Assist with your safety and health outcomes
- Manage chronic illnesses
How Can Case Management Help You?
The program can provide you with:
- Guidance: Health advisors understand the health care system and they can help you make the most of your benefits. Knowing when and where to go for care may save you time and money.
- Support: Your Health advisor will help keep you and your health care team talking. Health advisors know what questions to ask, where to find the answers and how the pieces fit together.
- Knowledge: You will be able to ask questions. We will give you general information about your health issues, medical bills and statements, medications and more. Plus you'll get news and tips about your treatment.
Many health conditions can be managed through our online Connect to Care program. It allows for tracking of health goals, connecting with clinicians via digital options, tips and more all in one place.
Who Is Eligible to Join and How Can I Get Started?
Members with a qualifying health condition may receive an invitation through email or text message. A clinician may also reach out by phone to invite you. If you receive an invitation, it will include instructions on how to sign up and start your program.
Referrals to Health Management Programs can also be made by members or caregivers, providers/practitioners, hospital discharge planners, or other medical management programs.
This is a voluntary, no-cost program and you can opt out at any time. It is not a substitute for the care of your doctor.
