This notice is required by California law and explains your rights under CCPA. CCPA applies to California residents only and does not apply to information collected from health plan members or applicants. Members should refer to the Privacy Practices Notice above.
Consent Management Portal
If you purchased your plan through the marketplace, you have the option to allow health information that we maintain about you to be accessed through a third-party online application (web app). If you agree to share your personal information with a third-party app, you can manage your consent at any time by using the Consent Management Portal.
Choosing an Application to Access Your Health Information
This notice provides helpful information about who is eligible to access certain types of data through an online application (web app) that you use. If you have questions, or if you are an Authorized Agent, Legal Guardian or Power of Attorney and have questions, please call the number on the back of your insurance card.
